Contractor / Subcontractor Application Package

Thank you for your interest in working with us. Please note, the below information must be completed and submitted prior to beginning any work. Also, we must have all paperwork on file before any payments are issued. Should you have any questions or concerns, please feel free to contact us at 1.888.502.5326.

The following information is required, by all contractors / subcontractors:

  1. A copy of the information package with each page initialed and the last page signed.
    A copy is available for download here.
  2. A completed Form W-9. This form is requred by law to prevent backup withholding.
    A copy is available for download here.
  3. A copy of your Worker’ Compensation premium certificate.
  4. A certificate of liability insurance withe Carrara Companies named as the certificate holder. Coverage requirements are listed in the information package.
